School Physicals and Well Child Exams

On May 7th and May 8th, the Ness City Medical Clinic will offer School Sports Physicals/Well Child Exams for the 2025-2026 school year for all current 6th-11th grade students. Any student wanting to participate in KSHSAA school athletics will need a physical completed and on file with the school before the first day of sports practice (See the attachment below for more information from the clinic).
These exams will be done during the school day and the school nurse will be in charge of getting the students to and from the medical clinic unless the parent would like to take them. Students will not be allowed to drive their own vehicles.
If you are interested, please have your student return the completed forms to the school or the medical clinic. For any questions, please contact the school or medical clinic. Please complete and turn in a KSHSAA physical form, Concussion & Head Injury Information Release and a Bright Futures Questionnaire. (see attached forms)
